Urodziła się i wychowała w rodzinie muzyków w Teksasie. Legenda rodzinna głosi, że najpierw nauczyła się czytać nuty, a dopiero potem litery. W wieku pięciu lat grała już na akordeonie, a sześć lat później jej biegłość gry na gitarze wzbudziła uznanie samego Cheta Atkinsa. Jako nastolatka występowała w grupie akompaniującej Johnny'emu Cashowi, Redowi Foleyowi i Texowi Ritterowi. Wielkie zainteresowanie wzbudzała jej gra na saksofonie i gitarze pedałowo-suwakowej w zespole rodzinnym The Mandrells u boku ojca Irby'ego i matki Mery Ellen. Zespół koncertował w USA, Kanadzie i Azji aż do 1963, kiedy to Mandrell nagrała swoją pierwszą piosenkę Queen for a Day. Po krótkiej przerwie, związanej z macierzyństwem, podpisała kontrakt wydawniczy z Columbia Records (w 1969) i zdobyła popularność nową wersją piosenki Otisa Reddinga I've Been Loving You Too Long.
W następnym roku wydała singla z piosenką Playin' Around with Love, która znalazła się wśród pierwszych 40 utworów na liście przebojów country i rozpoczęła owocną, długoletnią współpracę z wokalistą Davidem Houstonem. Duet ten osiągnął kilka znaczących sukcesów piosenkarskich.
W 1975 przeniosła się do ABC/Dot Records i rozpoczęła serię nagrań doskonałych piosenek, które osiągały z łatwością wysokie miejsca na listach przebojów. Były wśród nich: Standing Room Only, Sleeping Single on a Double Bed, (If Loving You Is Wrong) I Don't Want to Be Right, Years, That's What Friends Are For, Hold Me, Love Is Thin Ice, Waman to Woman.
W 1980 odbyła się premiera cyklicznego programu telewizyjnego w NBC, zatytułowanego Barbara Mandrell and the Mandrell Sisters, w którym występowały również jej siostry Louise i Irlene Mandrell oraz liczni zaproszeni muzycy. Każdy program kończyło wykonanie przez Mandrell utworu gospel, co w 1982 zaowocowało nagraniem płyty z tymi utworami, zatytułowanej He Set My Life to Music.
Po kuracji strun głosowych Mandrell wystąpiła w 1983 w spektaklu muzycznym w Las Vegas oraz nagrała dwa kolejne albumy - In Black and White oraz Spun Gold. Następnego roku przygotowała z Lee Greenwoodem płytę Clean Out. Wypadek samochodowy, w którym została (wraz z dwójką dzieci) ranna, wyłączył ją na cały rok z pracy. Kiedy wróciła do studia nagrań i na scenę, muzyka country na tyle się zmieniła, że artystka nie zdołała odzyskać dawnej popularności. Wówczas skoncentrowała się na działalności koncertowej. W 1990 opublikowała autobiografię - Get to the Heart: My Story.
